**Telemonitoring and Heart Failure Management: GPI and ASP of Agrigento Together for a More Innovative Healthcare System**

Blog, PHM

The project aims to revolutionize the approach to care for patients with heart failure, with the goal of easing the burden on hospitals and ensuring continuity of care.

The epidemiological framework, characterized by a 50% mortality rate within five years for cases not adequately managed, calls for a strategic reflection on the methods of taking care of these patients.

In this context, the remote monitoring project developed by the ASP of Agrigento, with the support of GPI, comes into play. This initiative will be presented from October 24th to 27th at the Innovation in Healthcare Technology Fair in Catania.

The project aims to revolutionize the approach to care for patients with heart failure, with the goal of easing the burden on hospitals and ensuring continuity of care, even remotely.

Benefits and Prospects of Telemonitoring

  • Greater autonomy for patients: fewer hospitalizations, more time at home, and a better quality of life.
  • Personalized care: each patient receives a tailored care plan, based on their specific needs.
  • Early interventions: continuous data collection allows early detection of potential deteriorations and timely interventions.
  • Cost reduction: fewer hospitalizations mean lower costs for the healthcare system.

Digitalization and “Connect Care”

Remote monitoring, facilitated by cutting-edge digital tools, allows patients to be constantly monitored from their homes, drastically reducing the need for hospital admissions and visits. Through “Connect Care” platforms, personalized care can be provided, improving not only the quality of care but also the patient experience. This approach enables greater accessibility to care, making it available even in geographically disadvantaged areas.

A New Approach to Care and the Patient Experience

Remote monitoring represents a true revolution in the management of heart failure, not only improving prognosis but also ensuring a better quality of life for patients. The new approach is no longer just reactive but proactive and predictive: thanks to the analysis of real-time data, it is possible to intervene before the clinical situation worsens, preventing complications.

Technology and Sustainability in the Healthcare System

The collaboration with the ASP of Agrigento demonstrates how GPI’s know-how, systems, and technologies can contribute to the development of a more sustainable and efficient healthcare system. The adoption of solutions such as remote monitoring reduces not only hospitalization costs but also the strain on healthcare resources, while simultaneously improving patient care.

  • Significant reduction in inappropriate hospital admissions
  • Optimization of healthcare resource utilization
  • Improvement of clinical outcomes
  • Rationalization of management costs

The Innovation in Healthcare Technology Fair in Catania will be an opportunity to present this innovative model to the public, marking a turning point in the management of chronic diseases in Sicily.
